• You are eligible for rewards after you complete health-related activities throughout 2023. Please note your Health-Related Activities must be completed in year 2023. You have until April 30th of 2024 to redeem all rewards earned during this benefit year.

• Your doctor or health care provider will send us your health-related activity information automatically. Please allow time for this process, once we receive the information from your provider, it will be processed automatically and you will receive the reward of your choice.

• More information on the Health Risk Assessment Health action details and how to complete this healthy activity.

• You are eligible for a $25.00 Reward for completing the Health Risk Assessment. The Health Risk Assessment is a survey you will complete that will help your health plan identify your health care needs.

• To complete the health risk assessment, you have a few different options. Please note the $25 reward will be automatically loaded onto your account after the health risk assessment is completed. You can complete the health risk assessment by:
              (1) Logging into your member portal at myhealthyrewards.healthmine.com.
              (2) Or by completing the paper copy that was mailed to you if you received one. The                        paper health risk assessment can be mailed to:
