Physical Gift Card Status - Incorrect Address

NOTE: MSR to check Rewards and Preference History tab to verify and advise the member when the rewards were mailed and the address they were mailed to.

  • We do apologize your reward was sent to the wrong address please allow me to confirm your mailing address to have your same gift card re-issued to your confirmed address. Please allow 3 weeks for delivery of your re-issued gift card, and we will provide you with a follow up call once it has been re-issued.

  • Before we end the call, please allow me to transfer you to Blue Care TN to have your address updated with your health plan. Please hold, I will introduce you to the Customer Service agent.

    • If member has expressed dissatisfaction during the phone call, Healthmine MSR warm transfers the phone call to BlueCare Plus Customer Service 1-800-332-5762 to handle escalation/complaint/grievance and update the member address on file.

  • Gift Card Status- <ENTER DATE & TIME>. HIPAA VERIFIED. MEMBER ASKING FOR STATUS OF GIFT CARD. GIFTCARD WAS MAILED ON <INSERT MAIL DATE> FOR <INSERT GIFTCARD VENDOR>.  THE GIFT CARD WAS MAILED TO THE INCORRECT ADDRESS. VERIFIED MEMBER ADDRESS IS <ENTER MEMBER MAILING ADDRESS>. A REQUEST WILL BE SENT TO RE-ISSUE <ENTER VENDOR NAME> ORDER FROM <ENTER ORDER DATE> ORDER NUMBER <ENTER ORDER NUMBER> TO CONFIRMED ADDRESS OF <ENTER MEMBER MAILING ADDRESS>. PLEASE ALLOW UP TO 3 WEEKS FOR DELIVERY AN OUTBOUND CALL BACK WILL BE MADE TO THE MEMBER ONCE THE GIFT CARD HAS BEEN RE-ISSUED TO THE ABOVE CONFIRMED ADDRESS.
